Home from Hospital Support Worker

We are seeking an experienced Support Worker to assist older people from hospital discharge.

Location: Based at University of North Tees Hospital. There will be multi-location working across Stockton- on- Tees and consequently the post is designated as ‘essential car user’.

Hours: 37 hours per week. Flexible hours (evenings and weekend working will be required)

Responsible to: Registered Manager

Salary Scale: SCP 14 (£17,694)

Job Purpose: When older people come out of hospital, they can need more support than usual to help get them back on their feet. Five Lamps’ Home Safe Sooner project provides this support for up to 7 days post discharge from University Hospital North Tees. Our staff can help with anything from transport home; ensuring the heating works and the house is tidy; collecting shopping or prescriptions and liaising with a wide range of other services to reduce the chance that an older person will end up back in hospital. They can also provide a vital link between the older person and the outside world at a time when they are still recovering from an accident or illness and encourage them to get involved in social inclusion activities

Contract Period: Temporary for 1 year starting on 2 October 2017. A permanent contract could be awarded subject to securing funding

Hours: Minimum contract of 16 hours per week. Flexible hours (evenings and weekend working will be required)

Salary Scale: £7.50 per hour (National Living Wage). Mileage, travel time and additional rates for bank holidays are paid in addition.

We are currently recruiting:

Enthusiastic and creative Volunteer Activities Support Workers

This role presents a unique opportunity to join an award winning charity, that has developed a new service and places older people at the heart of everything we do, ensuring best practice in meeting individual needs and the provision of outstanding care.

You’ll have an empathy with the needs of older people and understand the importance of treating our Service Users with dignity and respect. You will also have a sense of fun and a natural ability to help them make the most out of everyday life.

We want to give older people opportunities to meet up with old friends and make new ones while having some fun and sharing common interests. Lunch clubs bring older people together to enjoy a hot nutritious meal and socialise with friends at the same time. Lunch clubs may play host to music, bingo or guest speakers, but we will actively encourage volunteers to speak to service users and design sessions together and develop friendly and respectful relationships.
